Reforming markets in health care: An economic perspective

There has been an international move towards the creation of explicit
markets in health care, in which the purchase of care is separated from
provision. While the creation of such markets has undeniably led to
improvements in certain aspects of health care, it has also raised
important issues that have yet to be resolved - for example, is an
escalation of management costs an inevitable consequence of the
introduction of a market in health care? What sort of information is
needed to make the market function efficiently? Can a market-based
system be compatible with society's objectives relating to equity and
solidarity? The UK government is introducing reforms to the internal
health care market in the UK National Health Service which seek to
address concerns such as these, and this book comprises a series of
commentaries on their plans from a group of leading health economists.

BAXTER ALLEGIANCE PRIZE FOR HEALTH SERVICES RESEARCH RECIPIENT ANNOUNCED
Karen Davis, PhD, renowned health economist and president of The
Commonwealth Fund, has been named winner of the 2000 Baxter Allegiance
Foundation Prize for Health Services Research. The Prize is the highest
research honor in the field of health services.
The $25,000 Prize, funded by The Baxter Allegiance Foundation and
administered by the AUPHA, will be presented at the Association's Annual
Meeting on June 22-25, 2000, in Los Angeles. The Baxter Allegiance
Foundation is supported by Deerfield-based Baxter International Inc., and
McGaw Park-based Allegiance Corporation.
The award honors Dr. Davis for her tremendous contributions to health
services research, particularly in her role as a leader in national health
policy and her scholarly analysis of access to health care among
disadvantaged populations. Before joining the Fund, she served as chair of
the Department of Health Policy and Management at the School of Hygiene and
Public Health at Johns Hopkins University, and held an appointment as
professor of economics. Dr. Davis served as deputy assistant secretary for
health policy in the Department of Health and Human Services in the late
1970s, and was the first woman to head a U.S. Public Service agency.
With this award, the Commonwealth Fund plans to make a $25,000 award to
improve preventative and primary care for low-income working families in New
York City.

Opportunity for Post-doctoral Fellowship
In partnership with the Vancouver/Richmond Health Board (V/RHB), the
Centre for Health Services and Policy Research (CHSPR) at the University
of British Columbia (UBC) is seeking an individual with outstanding
training and/or research experience for a post-doctoral fellowship
position. Applicants must have a PhD (or equivalent) or an MD or other
professional health sciences degree and equivalent research experience.
Applications will be accepted from individuals with training and/or
research experience in the public health sciences including
health services research, epidemiology, biostatistics, health policy and
management, or related disciplines. Individuals with Masters Degrees and
particularly strong research backgrounds may be considered.
The research position is designed to assist the V/RHB in developing
programs for individuals with chronic disease. The precise conditions to be
studied and the contribution of these objectives to the overall research
will depend on the interests and skills of the successful applicant, the
policy needs of the V/RHB, and the overall research program of CHSPR.
Experience with the management and statistical analysis of large
administrative databases is essential. Individuals should have a record
of independent research and be able to work collaboratively
with the researcher and policy making communities. The ability to
communicate effectively is also required.
The post doctoral appointment will be for two years. Salary will

HEALTH ECONOMIST
The Rollins School of Public Heath of Emory University, Atlanta, Georgia,
has an immediate opening for a health economist in the area of vaccine
development. The appointment will be in the Department of International
Health. We are seeking a professional who has an established record of
independent research on vaccine development, randomized controlled
trials, and economic evaluation in the European or other international
health care settings. The Department of International Health offers
the opportunity to work closely with colleagues who have ongoing
projects in infectious diseases and health policy throughout the
developing world. Emory's Woodruff Health Sciences Center (which
includes the schools of Public Health, Medicine, and Nursing, the
Yerkes Regional Primate Research Center, several hospitals, and the

GENERAL ACCOUNTING OFFICE
Positions in Health Financing and Public Health, General Accounting Office,

We expect to have the following openings on the team, led by two senior
researchers, that evaluates large-scale demonstrations:
Quantitative Analyst/Quality Specialist. In the DOD Medicare Subvention
demonstration, Medicare pays DOD to provide health care for military
retirees in DOD's version of Medicare managed care. Issues of central
concern to policy makers are whether the care these retirees receive
is of comparable quality to the care received by other Medicare retirees;
and whether the demonstration has caused any deterioration in the
quality of care received by the DOD retirees. The person selected
will have primary responsibility for conducting the analysis and writing
a report. We expect that DOD care will be compared to national
benchmarks, using administrative data, and a panel survey will allow
analysis of the effect of the intervention on access, satisfaction,
and health status.
The ideal candidate will have knowledge of current methods for measuring
quality and will have strong quantitative skills, including SAS, Stata,
or a similar statistical package and experience in analyzing large
datasets. However, candidates will be considered who lack a quality
of care background but who have (1) experience with large datasets,
(2) good programming skills, and (3) sufficient statistical skills to
carry out regression-based risk adjustment of quality indicators.
Salary: $51,317-$93,626, depending on qualifications and experience.
Managed Care Specialist. Under the DOD Medicare Subvention demonstration,
Medicare pays DOD a discounted capitated rate to provide health care to
Medicare-eligible retirees. DOD is held to most Medicare+Choice
requirements. In evaluating the demonstration, it is important to
compare DOD's sites with Medicare+Choice plans along several dimensions,
e.g., quality of information, utilization management, quality assurance.
The person selected will be responsible for developing a design and
gathering data (e.g., interviews) from selected managed care organizations
for comparison with the six subvention sites. A large amount of
quantitative and qualitative data is available on the subvention
sites, but it may also be necessary to supplement these data with
data from comparable Medicare+Choice plans.
The ideal candidate would have both research experience and hands-on
managed care experience.
Salary: $51,317-$93,626, depending on qualifications and experience.

MICHIGAN POPULATION FELLOWSHIP
IPS Health Policy Programme (IPS HPP) is searching for a recent
post-graduate in the area of population and economics. The person would
be recruited to the staff of Michigan University, and placed at IPS for
for 1-2 years through Michigan's Population Fellows Program. Our
preference is for a recent PhD graduate in demography/economics.
Applicants must be US nationals or permanent residents. The
USAID-funded compensation package will be similar to other Fellows
in Michigan's Population Fellows Program.
The Population Fellow will be expected to allocate 20-25% of their time
to working with the Department of Demography, University of Colombo, and
at the Health Economics Unit, MOHFW, Dhaka, Bangladesh. The Fellow would
be expected to develop a research agenda in Sri Lanka consistent with IPS
HPP's own research goals. The unit has an interest in the policy
challenge imposed by Sri Lanka's rapid demographic ageing and the
financing of reproductive health care. It has working links with

The Centre for Health Economics and Policy Analysis(CHEPA) is a
university-based research organization whose mission is to foster
excellence in acquiring, producing, and communicating socially relevant
knowledge in the fields of health economics and health policy analysis to
inform health decision making. The Centre is seeking a Staff Health
Economist to conduct and manage health economic research on Centre
projects that are of high priority to the Ontario Ministry of Health and
Long Term Care, as well as providing advice and consultation on health
economic issues. The incumbent will: coordinate activities associated with
these projects; manage communication with individuals from the Ministry
of Health and other relevant organizations; conduct data analysis; manage
budgetary matters; write reports, and present research findings to external
audiences. The position also includes some administrative responsibilities
related to the Centre programmes and activities, e.g., assess the Centre's
hardware and software needs and ensure that the Centre's computing
resources are maintained. A Master's training in health economics and a
strong interest in policy-oriented research is essential. Excellent skills
are required in the following areas: both written and oral communication;
interpersonal interaction; critical conceptual and theoretical analysis;
data analysis including:
applied econometrics, and knowledge of common statistical software.

Novartis Pharmaceuticals Corporation invites applications for a
postdoctoral fellowship in health economics and outcomes research.
This is a unique opportunity for an individual who has just completed a
doctorate program (Ph.D., D.Sc., Dr.PH) in health services research,
public health, epidemiology, applied economics, or a related quantitative
discipline to gain practical outcomes research experience in the
evaluation of pharmaceutical products and programs.
The fellowship will enable a new scholar to sharpen his or her analytic
skills, learn to communicate research results, and apply learned skills
in the pharmaceutical industry. The fellow will participate in and
contribute to the research agenda of the Health Care Management (HCM)
department. Primary areas of research include health economics,
outcomes assessment, health care utilization, patient preferences,
compliance with treatment, and practice pattern variation. The
primary mode of learning will be active collaboration with researchers
in HCM on a variety of methodologic and applied topics in outcomes
research. The fellow may also audit courses and workshops in
economic evaluation and outcomes assessment.
Applicants must have completed a doctoral program in a relevant
discipline and must possess exceptional research and analytic skills,
and the ability to manage multiple concurrent tasks. Competency in
a computer statistical package such as SAS, STATA, or SPSS is a
must, as is experience in the management of large databases. A
clinical background and knowledge of managed health care is a plus.
The fellowship is full-time for a period of one year and will be
based at Novartis in East Hanover, NJ. The fellowship is not
intended for candidates who have not received their doctoral degree.
Start date is flexible, but the deadline for applications is May 31,
2000. Funding includes a $53,000 stipend and a relocation allowance.

ST. GEORGE'S HOSPITAL MEDICAL SCHOOL
An enthusiastic researcher is required to conduct one of three strands of
a research project to evaluate a unique service development for
personality disorder within the NHS. The economics strand will assess the
cost-effectiveness of the development and delivery of an existing
specialist psychiatric service and two new centres (London, Midlands and
the North West respectively).
The post-holder will work in close liaison with other research fellows
working on the project under the supervision of Mr Nicholas Graves at The
London School of Hygiene & Tropical Medicine.
The post is for three years and applicants must possess a postgraduate
degree in health economics or related discipline. Experience of
conducting economic evaluation within the NHS is desirable. Applicants

The University of Wisconsin Program in Population Health is offering a
post-doctoral fellowship to join in research with Program faculty and to
develop individual research. We are particularly seeking an individual who
can contribute to one or more of the following research areas: Bayesian
modeling in health services research; health outcomes and quality of life
measurement; cost-effectiveness analysis of health care interventions cost
modeling for assessment of new medical technologies; methods development to
summarize aggregate population health; simulation modeling applied to
population health issues.
Individuals holding a doctoral degree with training in statistics,
