PRIMARY CARE AND THE NHS REFORMS: A MANAGER'S VIEW
By Robert Royce

Robert Royce (former executive director of a UK health authority,
responsible for primary care services) analyses from a manager's
perspective the strengths and weaknesses of the current, primary care
led, reforms of the UK National Health Service. He argues that although
primary care physicians (`GPs') are central to the success of the reforms,
they currently have neither the incentives nor the full range of necessary
skills to make the reforms work.

He considers two future scenarios for the National Health Service.
In the first, GP practices would bid for franchises to deliver health
care services. In the other, there would be increased use of salaried
GPs, controlled by their employing Trusts.


NARROWING THE GAP BETWEEN PROVISION AND NEED FOR MEDICINES IN DEVELOPING
COUNTRIES
By Hannah Kettler

Clear, credible incentives are needed to motivate pharmaceutical
companies to invest more in research and development (R&D) for medicines
to treat diseases that primarily affect less developed countries (LDCs).

Hannah Kettler (OHE) argues that a combination of "push" and "pull"
incentives is needed. "Push" mechanisms reduce expected R&D costs;
"pull" mechanisms, such as a guaranteed purchase fund, increase
expected returns.

Public and private resources can be combined to achieve this. Two
recent initiatives provide examples of this: the Global Alliance for
Vaccines and Immunisation, involving the Bill and Melinda Gates
Foundation; and the Medicines for Malaria Venture combining public
funds and pharmaceutical industry inputs in kind (laboratory space,
access to combinatorial libraries, etc.) with academic research units.


RISK ADJUSTING HEALTH CARE RESOURCE ALLOCATIONS - THEORY AND PRACTICE
IN THE UK, THE NETHERLANDS AND GERMANY
By Adam Oliver

Adam Oliver (OHE) describes the risk adjustment mechanisms used in the
UK, where health care purchasers are non-competing, with particular
emphasis on England. He assesses how far the English risk adjustment
mechanism has reached the stated objective for its introduction: to
promote equal access to National Health Service health care for equal
need. The health care financing and risk adjustment mechanisms in two
countries that have competing health care insurers - The Netherlands
and Germany - are also described. The extent to which these mechanisms
have reduced the incentives for risk selecting is then discussed.

He concludes that the risk adjustment mechanism used in England has
moved the system there closer to the principle of equal access for
equal need. Concerning The Netherlands and Germany, he argues that the
risk adjustment mechanisms there take account of too few risk factors
to remove all, or even most, of the incentives for insurers to risk

select.


GENOMICS, HEALTHCARE AND PUBLIC POLICY
Edited by Paul Williams and Sarah Clow

This book is based on papers delivered at an OHE conference in February
1999. Contributors include Professor John Durant of the Science Museum
and Imperial College London on public understanding, Professors Sir
Michael Peckham and Sir Mark Richmond from University College London
on the impact on health care and the pharmaceutical industry, Professor
Everett Mendelsohn of Harvard on public policy making and the Iceland
experience, Professor Bartha-Maria Knoppers of the University of Montreal
on the ethical issues, and Professor Patricia Danzon of The Wharton
School on the economics of genomics. Main conclusions were:

* there is strong public support for the use of genomics to achieve
significant medical benefits, and risks are accepted, but in other
areas moral concerns arise and even low levels of risk may be
unacceptable;

* an international effort is needed to upgrade medical confidentiality
legislation to protect people from misuse of data, and so ensure the
continuing participation of individuals in genetic and genomic research;

* genotyping and genetic testing could bring great health benefits in
terms of diagnosis and treatment, with profound effects on the practice
of medicine.

Dialogue between industry, governments, patient groups and the medical
profession is needed to realise these benefits;


* the full potential of gene therapy may not be realised for those
suffering from hitherto incurable diseases because there may not be
enough patients to make commercial development economic. Orphan drug
legislation will help but other measures may be needed.

DPhil STUDENTSHIPS IN HEALTH ECONOMICS

Applications are invited for a three year studentship funded by the
Centre for Health Economics (CHE). The successful candidate will
register for a DPhil in the Department of Economics and Related Studies
(DERS).

The value of the studentship includes a maintenance award at standard
ESRC rates for a single individual and payment of University tuition
fees. In addition, as the studentship progresses, there may be
opportunities to work on research topics relevant to the thesis topic
and designated by the leader of the research team to which the student
is attached. This work will be paid, at rates equivalent to Research
Fellow 1B scales, according to the amount of time spent on it.

Candidates should have or be about to complete a Masters degree in
Economics or in Health Economics. The DPhil thesis topic must be in the
field of Health Economics, as defined by the Directors of CHE and of
the M.Sc. in Health Economics.

The successful candidate will be attached to one of the research teams
in CHE and their thesis topic will be expected to be relevant to the
work of the team. They will be provided with computing and other
facilities in CHE and will be expected to play a full role in the
intellectual life in CHE.

reports and presentation materials.
5. To assist the Manager, Population/Workforce Studies, in

b) coordinating the systematic review information
contained within a range of academic and non academic research

review projects;
d) maintaining a database registry of research
publication sources and detailing documentation of systematic review
procedures; and
e) designing and creating tables, charts and graphs and
other supporting documents for systematic review projects and
contributing o the preparation of preliminary drafts of research
reports;
2. To assist the Manager, Population/Workforce Studies, in
managing some of the day-to-day activities of the research program,
including but not limited to:
a) developing and monitoring project budgets and time
lines for projects involving the research associate; and
b) preparing the agendas and minutes of research project

EDUCATION AND EXPERIENCE
Qualified candidates will have a demonstrated proficiency in
bibliographic research methods, the use of computer database
applications, excellent oral and written communications skills and
strong organizational talents. Candidates with strong skills and
training in the critical appraisal of research literature and
familiarity with research methods will be especially suited to this
position. Qualified candidates may have a masters degree in
epidemiology, economics or other health or social science discipline.

of the Kazakhstan country program strategy and workplan.

2. Coordinate with all regional technical advisors to meet Kazakhstan
technical assistance needs in all key technical areas, including
finance and economics, clinical policy, integrated primary care, and
community involvement.

3. Lead overall policy dialogue with government officials and
counterparts at the national and regional levels in Kazakhistan.

4. Supervise the Kazakhstan site coordinators.

Qualifications:

1. PhD or equivalent experience in health economics, health
financing, or related field. Specialty in macrofinancing issues
related to health care desirable (e.g. taxation, designing and
developing national health insurance systems, national health
accounts).

2. Minimum of 10 years experience in the field of health economics or
health financing. Significant experience designing and implementing
health financing policies in either developed or developing country
context.

3. Experience managing and supervising staff in a multi-cultural
environment.


4. Experience in NIS or Eastern Europe desirable.

5. Ability to work in Russian language desirable.

6. Experience working on USAID projects desirable.

Job description

General: An experienced health economist is required for a full-time
post, funded for 2 years in the first instance, within the Health
Economics Research Centre (HERC), University of Oxford. It is expected
that the successful applicant will have a postgraduate qualification in
health economics and substantial work experience in a recognised research
centre. This post is funded from research grants and awards held by
HERC, and is available immediately.

HERC is part of the Department of Public Health and is located within
the Institute of Health Sciences, Headington, Oxford, approximately 1.5
miles from the city centre. The Division of Public Health and Primary
Health Care was rated 5* in the last RAE. The membership of the Institute
of Health Sciences embraces a number of leading research groups including
the Centre for Statistics in Medicine, National Perinatal Epidemiology
Unit, ICRF Cancer Epidemiology Unit, Oxford Institute for Ethics and
Communication in Health Care Practice (Ethox), Health Services Research
Unit and Centre for Evidence Based Medicine. HERC also has extensive
links with other parts of the University of Oxford Medical School.

HERC
HERC obtains core funding from the NHS Regional R&D, with additional
posts funded from project grants. At present it consists of a Director
(Dr Alastair Gray), 3 Research Fellows (Dr Philip Clarke, Ms Kathy
Johnston, Ms Jane Wolstenholme), 1 MRC Training Fellow (Dr Andrew Briggs),

1 visiting Senior Fellow (Ms Karen Gerard), an Administrator (Ms Alison

Gater), and 4 Research Associates. Three doctoral students and one
Chevening scholar are also based within HERC at present.

Since its formation in 1996, HERC has been committed to its development
as a centre of excellence in health economics in the University of
Oxford. It maintains good links with other health economists and
health service researchers locally, nationally and internationally,
and with purchasers and providers in the National Health Service.
Staff are involved in a wide range of applied and methodological research
projects, and in supervision, teaching and consultancy.

At present HERC is involved in a number of major evaluations, including
the UK Prospective Diabetes Study, the MRC International Subarachnoid
Aneurysm Trial, the MRC Spinal Stabilisation Trial, and NHS R&D HTA
programme Knee Arthroplasty Trial (further details of these and other
current research are on the web site - www.ihs.ox.ac.uk/herc).

New projects include a randomised assessment of the relative
cost-effectiveness of new and old drugs for Parkinson's disease, funded
by the NHS R&D HTA programme and a trial of the cost-effectiveness of
a home visiting parent support programme, funded by the DH. These
studies afford many opportunities for developing analytic techniques
and methodological research. HERC is also involved in decision
analyses and other modelling studies. Current modelling studies,
commissioned by the UK National Screening Committee, include screening
for prostate cancer and screening for hearing impairment. In addition,
staff have a range of other research interests, including international
comparisons and health inequalities.

The post HERC is now seeking to appoint a Research Fellow (RS Grade 2).
The successful applicant will take responsibility for a number of new
research projects and will be encouraged to maintain and develop their
existing research interests and to generally play a full and enthusiastic
role in the work of HERC. The new areas of research the post-holder
will take responsibility for include the cost-effectiveness of drugs for
Parkinson's disease and the cost-effectiveness of a home visiting parent
support programme.

It is also expected that the successful applicant would initiate new
research projects that promote the main areas of interest of HERC, and
obtain funding for these where necessary; would respond to requests for
research advice and assistance; would play a play a part in maintaining
and expanding the current teaching activities of HERC; would provide
advice and supervision to research students and junior colleagues; and
would promote and disseminate the work of HERC by means of peer reviewed

Background/Project Objective(s):

The Multsectoral HIV/AIDS Prevention Programme (MAPP) under the
overall supervision of the Ministry of Health and Family Welfare and
with funding from the United Nations Development Programme (UNDP)

and STD related issues, the Strategic Plan and the National Action

THE PROGRAMME: This series of workshops is intended to provide a forum
for the development and dissemination of applications of econometrics and
other quantitative methods in health economics. The focus is on applied
econometrics but we are happy to consider health-related applications

of mathematical economics, experimental economics and other quantitative
approaches.

As in previous years, the number of participants will be limited to
around 40 to avoid the need for parallel sessions. The programme will
consist of 12-15 papers. Each paper will be allocated an hour, made
up of a short presentation by the authors, followed by a response from
a nominated discussant and a general discussion. Papers will be
circulated in advance and taken as read. Participants will be expected

to attend the whole meeting and it is hoped that all participants will
play a role as either author, discussant or chairperson.

This meeting is part of a series, Euroconferences on Econometrics and
Health Economics and is funded by the Human Potential Programme of the
European Commission. Subsidies should be available to help cover the
registration and travel costs of those who participate.


SELECTION OF PAPERS: Papers for the 2000 Workshop will be selected on
the basis of full drafts. The deadline for submitting draft papers is
1 July 2000. Authors will have the opportunity to submit their papers
for publication in the `workshop issue' of HEALTH ECONOMICS, and it
would be helpful if papers are prepared according to the journal's notes

Joint International Conference: APHEN (Asia-Pacific Health Economics
Network) Forum 2000 and 5th Annual Conference of the Bangladesh Health
Economics Unit (HEU).

An international conference on Health sector reform: equity, efficiency,
sustainability? to be held 4 - 6th July 2000 in Dhaka (venue to be

The conference will bring together specialists working inside and
outside the Asian-Pacific region. The aim is to provide a forum in which

The 57th Health Economists' Study Group meeting will be held on 10 - 12
July 2000 at the Jubilee Campus, University of Nottingham.

This is an ordinary meeting with papers on any relevant topic.

Priority will be given to work-in-progress papers.

The format for sessions will be one hour, as standard, with written
papers submitted in advance, introduced and reviewed by a discussant
other than the author.

If you wish to present a paper, please send a title and abstract
(up to 250 words) of the proposed content, with some indication as to
the nature of the piece and techniques expected to be used.
DEADLINE FOR ABSTRACTS: Friday 14th April 2000

As agreed this information will be made available on the HESG web-site
to allow a more informed choice for those interested in discussing.
Deadline for submission of full papers will be Friday 9th June 2000.

Please submit abstracts to Paul Miller by email if possible, as text in
the body of the e-mail message.

Full papers must be submitted in Word 97 format and sent either by
e-mail attachment or on floppy disk. This is so they can be placed on
the HESG mailbase www site in downloadable format (further details on
this will follow).

For your information the meeting will take place over three days
starting at around 2pm on Monday 10 July and finishing after lunch on
Wednesday 12 July 2000, at the new Jubilee Campus, Nottingham University.
(NB. Not the campus at University Park).

As we plan on making all accepted abstracts available on the HESG
mailbase www site, please submit abstracts by e-mail, with the text
in the body of the message not as an attachment. Send one email per
abstract to Paul with a clear header that it is an `HESG abstract'
if you have no email send a paper copy.


If numbers of papers submitted exceeds the number of slots, standard HESG
selection criteria will apply ie:

1. HESG meetings are a forum for the discussion of health economics
issues using the language of economics; papers with no or a very small
economics content will be rejected.

2. Work in progress papers, those at an early stage and those on
important health economics issues are welcome. Reproduced reports and
papers (especially those written for other audiences) and book chapters
will be rejected.

3. Authors, or sets of authors (but not institutions or units) may be
restricted to one paper.

4. Only abstracts submitted before the deadline will be considered.

Full instructions for authors will be sent out once papers are accepted.

economists, foster a higher standard of debate in the application
of economics to health and health care systems, and assist young
researchers at the start of their careers. Activities of the

- Present the annual "Kenneth J. Arrow Award for Best Paper in Health
Economics."


- Hold biennial conferences, the first May 19-23, 1996 in Vancouver,
B.C. (640 delegates attended), the second in June 6-9 1999 at
Erasmus University, Rotterdam, and the third July 22 - 25, 2001 at
CHE, York, England.


- Assist in the distribution of Journal of Health Economics and Health
Economics, and provide both publications at discounts to members.


- Publish a biannual newsletter, HEAL (Health Economics Analysis
Letters), and a monthly electronic newsletter, iHEA NEWS
listing upcoming meetings, recent paper presentations, jobs,
and professional commentary.


- Publish a series of books in health economics and a World Directory
of Health Economists listing individuals and organizations.


- Assist in the distribution of eHEL, electronic Health Economics
Letters, an electronic journal with internet dissemination
and peer-review of economic studies of policy, effectiveness,
technology, market structures and pharmaceuticals around the world.
