If the document is not well-aligned, reset your mail reader to display a non-proportional font (such as Courier). ------------------------------------------------------------------------- ========================================================================= FEATURE ARTICLES ========================================================================= RESEARCH ON HEALTH ECONOMICS IN LOW AND MIDDLE INCOME COUNTRIES ANNE MILLS, HEALTH ECONOMICS AND FINANCING PROGRAMME, LONDON SCHOOL OF HYGIENE AND TROPICAL MEDICINE The considerable increase in interest in the economics of health that has occurred over the last few decades in Europe and the US is paralleled by a similar increase in interest in low and middle income countries, though backed by fewer resources and more limited research capacity. The London School of Hygiene and Tropical Medicine has been active in this area since 1979, though with a considerable expansion since the creation of a Health Economics and Financing Programme in 1990. This is underpinned by a programme grant from the UK Department for International Development, which funds both research and capacity development activities overseas. The research of the Programme falls into a number of areas including overall health system reform; financing of health services; relationships between public and private health sectors; and economic evaluation. Current major research projects include: * study of both the content and process of health care reform (with two separate EU-funded projects, one in the Caribbean and one in Southern Africa) to identify lessons to do with how reforms are implemented and whether there are particular approaches that can be recommended * study of hospital reform in several countries, and in particular of issues associated with giving public hospitals greater autonomy in raising revenue and managing their funds * investigation of the feasibility of community-based health insurance in Ghana, and in India a study of the factors affecting the success of such schemes * several studies looking at the impact of different approaches to paying hospitals and doctors (in Thailand and China), with in Thailand a particular focus on evaluating capitation payment in the context of social insurance * evaluation of contracts between public and private sectors for the provision of health care: this work started with a number of studies of contracting out hospital services, and now focuses on evaluation of contracts for primary care in South Africa * mapping of the regulatory structure for health care in a number of countries, moving into a phase of evaluation of specific regulatory reforms * study of the nature of health care markets both for health care and for specific health-related commodities such as drugs, mosquito nets, and insecticide * a considerable collection of economic evaluation studies in the areas of HIV prevention, malaria, and reproductive health: a common theme in a number of these is use of a modelling approach to enable useful conclusions to be drawn from existing data. Projects are also addressing the need to improve the methodology of economic evaluation, particularly producing a better measure of health related quality of life for use as an outcome measure, and developing greater understanding of factors affecting the generalisability of cost-effectiveness results across countries. All our field research is done in close collaboration with country research groups, with particularly close research collaborations in South Africa (the Centre for Health Policy, University of the Witwatersrand; the Health Economics Unit, University of Cape Town), Zambia (the University of Zambia), and Thailand (the Health Systems Research Institute). We have a large PhD programme with the majority of students coming from low and middle income countries, who can help strengthen health economics research capacity in their countries on their return. Given our funding from the Department for International Development, we place great emphasis on ensuring that the results of our research are communicated both within the countries we work in and internationally. Since we are one of the very few major centres for research on health economics in low and middle income countries, we see one of our functions as ensuring that information about who is doing what is widely disseminated. Every few years we produce a directory of people working in health economics-related research in low and middle income countries. We produce a newsletter three times a year which provides information on research and journal and book publications. Short briefing notes on particular research results are also sent out with the newsletter, and we produce a research portfolio which provides a complete listing of all our current research projects with short summaries and contact names. We also have a Departmental publication series to which contribute. Requests for further information and to be put on our mailing list for the newsletter and other Programme documents should be sent to nicola.lord@lshtm.ac.uk. --------------------------------------- HEALTH ECONOMICS AT CARNEGIE MELLON Health Economics at Carnegie Mellon Carnegie Mellon University has a long history of productive research in health economics -- e.g., the groundbreaking work on hospital cost functions by Judith and Lester Lave, and fundamental work on cost-effectiveness analysis by Mark Kamlet. In recent years, Carnegie Mellon has built on this history to develop a core strength in health economics research and education, with an emphasis on examination of competition and health care markets and institutions. Carnegie Mellon will be hosting the year 2002 annual health economics conference co- sponsored by the VA Management Science group. Health economics research at Carnegie Mellon focuses on three areas: (1) Industrial organization of health care -- Researchers working on issues in this area include Professors Martin Gaynor and William Vogt and graduate students Jean Abraham and Farasat Bokhari. This area of research includes work on hospital competition, technology adoption, insurance choice, antitrust policy in health care markets, and optimal incentives in health care organizations. (2) Cost-effectiveness analysis in health care -- Professor Mark Kamlet's research concentrates on the theoretical underpinnings of cost-effectiveness analysis and on application issues. (3) Health impacts of environmental regulation -- Professor Lester Lave heads a large group of researchers exploring critical issues related to health effects of environmental policy. A special advantage of the health economics strength at Carnegie Mellon is that it dovetails well with the universityís related, pre-existing strengths in: - applied microeconomics, - technology policy, - behavioral decision theory, - biomedical technology development, and - health informatics. The combination of the universityís complementary strengths and its interdisciplinary culture creates exciting opportunities for joint research resources and projects. For example, plans are underway for a medical database repository that would be a resource for researchers in both health policy and medical records confidentiality. The Carnegie Mellon Census Data Research Center, the first such center housed at a university, is a unique resource providing access to confidential Census data. Health economists at Carnegie Mellon also benefit from the strong ties that they maintain with health economics and health services researchers at the near-by University of Pittsburgh.
