The Editors would like to return to an active solicitation of Feature
Articles on on-going research by health economics research groups,
academic departments, and other organizations, ideally appearing in
each issue of iHEA News.
In particular, we will be soliciting Feature Articles from the
Corporate and Academic Institution members in iHEA. The first
article in this series below focuses on monographs available from
the Office of Health Economics. Working paper and monograph series
announcements are particularly encouraged. Descriptions of new
graduate degree programs by academic departments, governmental
policy initiatives, particularly innovative and informative web pages,
or other similar announcements are also desirable.

NEW PUBLICATIONS FROM THE OFFICE OF HEALTH ECONOMICS
The Office of Health Economics (OHE) was founded in 1962 and is based at:

The OHE's terms of reference are to: commission and undertake research
on the economics of health and health care; collect and analyse health
and health care data; disseminate the results of this work and stimulate

November and December 1998, as listed below.
Controlling NHS Expenditure: the Impact of Labour's NHS White Papers
- by Jon Sussex.
The UK government is on the brink of introducing major changes in the
way that the National Health Service (NHS) works. In this monograph,
Jon Sussex looks at the way two of these reforms may clash, namely:
abolishing GP fundholding; but bringing the cost of GPs' prescribing
within the fixed NHS cash limit for the first time. He concludes
that budgets and financial incentives for individual GP practices
may have to be re-introduced but, unlike the voluntary fundholding
scheme, these will have to apply to all practices.
GP Commissioning Groups - the Nottingham Experience
- by Dr Stephen Earwicker.
>From 1 April 1999, the purchasing of NHS care in the UK will, more
than ever before, be in the hands of GP-led groups: Primary Care Groups
in England, Local Health Groups in Wales and Primary Care Trusts in
Scotland. Valuable lessons can be learned from the experience of the
Nottingham Total Commissioning Project (TCP). Major issues that have
arisen and are discussed in the OHE Briefing include:
* management costs;
* keeping within budgets;
* relations between the TCP, the Health Authority and provider Trusts;
* lack of involvement of professional groups other than GPs.
Dr Earwicker concludes that the major achievement of the Nottingham TCP
so far has been to demonstrate that the GPs and their local Health
Authority can collaborate effectively.
Our Certain Fate: Rationing in Health Care
- by Professor Alan Maynard and Karen Bloor.
Whether called 'rationing', 'priority setting', 'resource allocation',
or anything else, the policy question is not whether resources have to
be rationed, which is inevitable however generously health care is
funded, but how. The authors compare the various principles that
might be used when rationing health care and describe the practice of
rationing as attempted in Oregon (US), The Netherlands, New Zealand,
Norway, Sweden, Finland and the UK. Health care policy makers in
other countries have confronted the rationing issue more explicitly
than in the UK. But although some have come to terms with the need
to ration on the basis of cost-effectiveness (e.g. in The Netherlands),
no country has yet properly translated rationing principles into
practice. Overall, Maynard and Bloor show that there is a need both
for explicit principles to be agreed to determine patients' access to
care, and for an adequate body of evidence about the benefits and
costs of different types of care.
Competition through Innovation, Innovation through Competition
- by Dr Hannah Kettler.
Policy makers face the seemingly conflicting tasks of keeping health
care costs under control while maintaining an environment which
supports and encourages investments in innovative research and
development (R&D) to produce new medicines. Many new medicines
offer incremental rather than breakthrough innovations. Some
politicians and academics have therefore discussed possible policies
to encourage breakthrough innovation by actively discouraging these
incremental innovations, which they view as poor value for money.
Hannah Kettler has reviewed the economics of pharmaceutical innovation.
As a result she concludes that such policies risk being
counterproductive, because:
* pharmaceutical R&D is a process with very uncertain outcomes.
Incremental innovations are often the result of R&D aimed at producing
breakthrough medicines. Seeking to discourage the former therefore risks
losing breakthroughs too;
* many incremental innovations provide economic and therapeutic value;
* the full value of many new medicines does not become known until after
they have been launched.
Biotechnology and the Economics of Discovery in the Pharmaceutical
Industry
- by Helen Simpson.
The biotechnology industry has in the last year attracted more negative
attention than positive, particularly in the UK. But biotechnology
firms are fulfilling an important role, especially in the discovery
of new medicines. Helen Simpson examines the economic reasons for
the emergence of biotechnology firms and their impact on the structure
of the pharmaceutical industry. Advances in technology were the
triggers, reducing the minimum efficient scale of organisation needed
to discover new medicines. The existence of a pharmaceutical industry
with an insatiable demand for such discoveries provides an active market
for research outcomes. This combined with a capital market, in
particular venture capitalists willing to finance high risk biotechnology
start-ups, has created a favourable environment. Together these factors
provided research scientists with a new opportunity to establish small
biotechnology businesses, outside existing, large pharmaceutical
companies and outside universities. Helen Simpson concludes that
small biotechnology firms will contribute to improved research
productivity because they provide better incentives for some,
entrepreneurial, members of the pharmaceutical research community.
However, the difficulties facing small companies when seeking to
develop and market their products, mean that they are likely to
continue to focus on the initial discovery stage. Large, integrated
firms are therefore likely to continue to dominate the pharmaceutical
industry. These large firms will also continue to undertake much
discovery themselves.
Tuberculosis
- by Nick Marchant.
Tuberculosis (TB) is the single largest infectious disease killer
in the world today. It causes three million deaths every year,
with South East Asia and Africa the worst affected regions. But
TB is not only an issue for less developed countries. After many
decades of decline, the numbers of new TB cases occurring in
industrialised countries are now starting to increase. Each
year there are now more than 6,000 new cases of TB notified
in the UK, and over 500 deaths from it. Nick Marchant sets out
what is currently known about TB and the effectiveness and cost
of its prevention and treatment. He assesses both the global
threat and the specific issues of preventing and treating TB in
the UK and concludes that success in fighting the disease in any
one country depends on tackling the epidemic world-wide.

The Robert Wood Johnson Foundation Investigator Awards in Health Policy
Research requests proposals to interpret, develop, or substantially
advance ideas or knowledge that can improve health or health care
policy in the United States.
Applications are encouraged from investigators in diverse fields,
including economics, sociology, political science, education,
anthropology, history, health and social policy, public health,
medicine, nursing, allied health, law, business, philosophy, ethics,
journalism, social work, and the management sciences. Applications from
individuals in non-academic settings, such as research firms and policy
organizations, also are encouraged.
Grants of $100,000 to $250,000 will be made, principally for project
salary support for up to three years. Deadline for receipt of letters
of intent is April 9, 1999.

JOHN D. THOMPSON PRIZE FOR YOUNG INVESTIGATORS
CALL FOR NOMINATIONS
Nominations are being sought for the 1999 John D. Thompson Prize for
Young Investigators, which recognizes young investigators based on their
contribution to the research literature in the field of health services.
The Prize consists of an honoraria and travel expenses to the 1999
Association of University Programs in Health Administration (AUPHA) Annual
Meeting. Funds donated by the Health Administration press and the many
friends of John D. Thompson form the Prize's endowment.
The Prize is awarded to an individual whose work may be in any
discipline contributing to knowledge in the health services. Emphasis is
placed on the applicant's publications in peer reviewed scholarly
journals. The judges may also consider books and other publications.
Nominees should have received their Ph.D. within the last 5 years. Anyone
may nominate a candidate. A nomination should include a current
curriculum vitae on the candidate, a single letter of not more than five
pages discussing the candidate's contribution and justifying the
nomination, and complete copies of the candidate's publications of which
the nominee is sole or first listed author. These must appear in print
before December 31, 1998. Individuals may be nominated more than once.

RESIDENT ADVISOR IN THE DOMINICAN REPUBLIC
The Partnerships for Health Reform Project (PHR), a USAID-funded health
reform project, is a five-year project ending in September of the year 2000.
The Project is implementing a major, long-term program in the Dominican
Republic in response to a request from the Executive Committee for Health
Sector Reform (CERSS) to USAID/DR for technical assistance to:
a) design a system to monitor and evaluate the health sector reform process,
results and impact and,
b) strengthen the management capacity of the provincial health directorates
(DPS).
These activities, to be developed during FY99, in addition to other requests
during FY00, will increase the capacity of the Dominican health sector to
provide, monitor and evaluate preventative care and primary health care
services.
The Project is currently seeking candidates for the position of Long-Term
Resident Advisor in the Dominican Republic. The resident advisor will,
in close collaboration with PHR Bethesda-based staff and consultants, CERSS,
Secretariat of Public Health and Social Assistance (SESPAS) and USAID/DR
manage and implement technical assistance to the Dominican Republic.

The incumbent will be responsible for the development of global economic

communication mediums. This includes the strategical and tactical economic
input to both clinical development and business colleagues from phase 0,
prior to "entry-into-man" until the product is launched in the major
markets. The role involves initiating and conducting studies on the burden
of disease and the cost to the economy and to patients. The creation of
economic and quality of life evaluation of drugs both alongside clinical
trials and through the use of modelling will be within the frame of
responsibility. The position is within a strong and ambitious team of
pharmacoeconomic project managers and experts within the global

To meet the requirements of this specialist position you need a degree
either in economics or life science ideally backed by a post graduate
qualification in business, marketing or health economics. The successful
candidate will have at least 2-3 years industrial/consultancy experience
within the pharmacoeconomics arena. Applicants with a background in
clinical development or marketing and with a substantial exposure to - and
knowledge of - health economics activities would also be considered.
Excellent communication skills are essential, together with good analytical

Senior Scientists, MRC Health Services Research
Collaboration
Applications are invited for up to 5 MRC senior posts
tenable, in the first instance, at the Medical Research
Council's Health Services Research Collaboration,
Department of Social Medicine, University of Bristol.
These positions are tenured MRC career scientists, whose
main roles will be to lead the major research programmes
within the HSRC, as well as contributing to the training

with international reputations in the field. It is
expected that the complete team will include individuals

a 15-day executive development program at the Stockholm School of Economics

The program runs under the responsibility of Prof. Bengt Joensson and
provides participants with in-depth knowledge of economic evaluation and a
better understanding of the international health care market. The
comprehensive course is designed primarily for managers in the
pharmaceutical industry, payers and providers in public and private health
care organisations and regulators. The residential program alternates
lectures, case studies and group work and is limited to 30 participants to
provide opportunities for discussions between participants and faculty. The
faculty consists of lecturers from academic institutions, payer and provider
