The theme of this Conference is: Private and Public Choices in Health and Health Care. Private choices of patients, health care providers and health care insurers are the driving force of all health care systems. They are also the subject of economic analysis of health care demand, health care production and health care financing. Public choices reflect the need to assure access to health care, and to correct for failure in the health care market and in the market for health care insurance. Economic analysis focuses on the incentives associated with various forms of market intervention; from regulation to financing to public provision of services. What is the optimal private/public mix in the health care sector? What can we learn from systematic comparisons of private and public choices across nations of different traditions and wealth? Which forms of private and public sector co-operation have proven to be successful, what new forms are emerging? How can we reconcile virtually unlimited opportunities to provide health care with positive effects on health with limited public means? These are some of the issues that will be addressed at the Conference. Furthermore, we will have a plenary session devoted to Economic Evaluation of Health Care Programs in both high and low income countries and invited sessions on methodological problems and policy issues in this area. We work together with the World Bank and the Economic Development Institute to bring together in Rotterdam as many colleagues as possible form Africa, Asia and South America and organize (pre-conference) sessions on relevant topics for low income countries, such as the development of National Health Accounts. The Conference will be held in the World Trade Center, in Rotterdam, a cosmopolitan city with daring architecture, where 17th century stately homes join hands with ultra modern sky scrapers and bridges. The World Trade Center itself is a startling example of the encounter between old and new: the old exchange building carries a brand new office tower on top of it's roof. The intent of this competition is to encourage the development and presentation of first rate papers by graduate students. Papers that are co-authored with thesis supervisors or other senior faculty will be discouraged, in order to be able to judge the student's work. The author of the paper judged to be the best, by the Student Competition Committee, will be offered a waiver of Conference registration fees, as well as return economy air fare and accommodation expenses during the course of the Conference. The development of capitation systems poses considerable methodological problems from both theoretical and empirical perspectives.Ý Moreover, these problems may vary depending on the type of health care expenditure under consideration and the type of health care system in force. The purpose of this special issue is to examine the theoretical basis underlying capitation, to review the variety of capitation systems now in existence, to describe empirical approaches used in developing capitation models, and to discuss the policy difficulties that arise in implementing capitation systems.Ý Submissions are therefore welcome from a variety of theoretical and empirical perspectives, and from different institutional settings.
