The post has been developed by a consortium consisting of Brunel University, Hillingdon Health Authority, and the Hillingdon Hospital NHS Trust. Funding for the post has been obtained from the North Thames NHS R&D programme. Within that, the general descriptive analysis of the data in the first round of the main collections, and the analysis of such key issues as who gets how much of what service, and the 'ological defensibility of patterns of utilization are taking precedence over the analyses dependent on the second round. The posts will be located within the Health Economics section of the School of Health and Related Research, a successful and growing multi-disciplinary centre established to conduct and promote health services research, and undertake teaching and consultancy. Applicants should have a first degree in economics and preferably a postgraduate qualification in health economics or related subjects but applications will be considered from good quality candidates wishing to acquire experience in the health care area. The holder of the Research Associate post will be specialising in trama care and working on several related studies in this area with colleagues in the Medical Care Research Unit. The Research Fellow would be significantly involved in contributing to a project reviewing evidence to assist in purchasing decisions and would ideally have several years experience in research or the NHS. Both postholders would have opportunities to work in other areas and to develop their own research interests. They will be members of an experienced team of health economists . Research Fellow The person appointed will work for half their time on the Working Group on Acute Purchasing. The aim of the Working Group to allow purchasers to share research knowledge about the effectiveness and cost-effectiveness of acute service interventions and to determine collectively their purchasing stance.. The Sheffield Unit of the Trent Institute for Health Services Research facilitates this Working Group, who's membership includes representatives from Public Health Medicine and Commissioning from purchaser organisations within the Trent Region together with Regional Office staff from Public Health, Commissioning and Research and Development. A list of interventions for consideration is recommended by the purchasing authorities in Trent and approved by NHS Executive Trent. A team from ScHARR provides the key analytical input into the assessment of the cost-effectiveness of the interventions, including health economics, literature searching, and modelling. A seminar is then led by the consultant on the particular intervention where purchasers and provider clinicians consider research evidence and agree provisional recommendations on purchasing policy. The guidance emanating from the seminars is reflected in a series of Guidance Notes for Purchasers, which will be approved by the Trent Development and Evaluation Committee (DEC), recently established along the lines of that running in the South and West Region. So far, seven interventions have been evaluated: Tertiary Cardiology, DNase, Beta Interferon, Bone Marrow Transplantation, Bone Anchored Hearing Aids, Cochlear Implants, Prostacyclin, the Statins, CT Scanning for Stroke Patients, Spinal Cord Stimulation in the management of Chronic Pain, Osteoporosis and Breast Cancer. The first document in the Guidance Notes for Purchasers series (The Use of DNase in Cystic Fibrosis) was published in March 1996. This was followed in June by a paper on the Role of Beta Interferon in the treatment of Multiple Sclerosis, which was issued in the Trent Institute Discussion Paper series. It is expected that a further eight or nine Guidance Notes will be produced during the year. The Research Fellow will be responsible for co-ordinating the economic input into the Working group. For many projects it will be appropriate to call on the the assistance of colleagues in order to benefit from their knowledge of an area. However the main input will come from this post. For the remainder of their time, the person will be able to work on research projects within the Health Economics section and would be encouraged to be involved in the process of designing studies.
